32 patients with hypogonadism were studied. They were suffering from hypospermatism, oligospermatism or azoospermia. LH-RH loading tests were performed. There is a parallelism between the follicle stimulating hormone and luteinizing hormone changes. After LH-RH loading, the reactions are similar in most cases: the levels increase in a parallel fashion although these increases are totally distinct. The luteinizing hormone curves are similar in all cases. There are clear differences however in the follicle stimulating hormone curves: in the case of normospermatism, the increase was 29-107% while with pathological cases the changes were proportonal to the degree of hypospermatism. It appears that when a low follicle stimulating hormone level is significantly increased by the LH-RH loading, the disorder is caused by a testicle abnormality. When the follicle stimulating hormone level does not increase after loading, a stimulation with clomiphen does not improve the situation. The LH-RH loading test has proved very useful in establishing the pathological mechanism of hypogonadism.
