Catastrophic presentation of lupus vasculitis in a young female

18-year-old female presented with a one hour history of altered sensorium and facial asymmetry. Patient had a history of joint pain mainly involving the large joints of both upper and lower limbs since last 1 year. There was no other significant history in the past. The endotracheal tube suction of the patient revealed bloody secretions suggesting pulmonary haemorrhage. Mechanical ventilation was initiated but in the next 2-3 hours her condition continued to worsen and finally within 36 hours of admission patient succumbed to her illness.
