The specific complex of symptoms known as postperfusion pulmonary syndrome are discussed and studies are reviewed of the intrapulmonary microvascular reaction in its etiology, including pulmonary hypoperfusion and hypoxia, the effect of homologous blood, microembolisms and humoral substances. Postperfusion pulmonary syndrome is apparently a polyetiological complication of open heart surgery; the main role is played by the intrapulmonary microvascular reaction in its 2 stages of periarterial and alveolar impairment. The 1st of these is reversible, but the 2nd is apparently not. The main component of this reaction can be considered hypertension in the pulmonary microvessels, while of all its etiological factors, the hardest to combat are hypoperfusion of the lungs, microemboli and the effect of vasoactive substances.
