The posterior jaw presents an array of challenges for the implant surgeon and restorer. Sinus and nerve position can limit the available aleolar bone height. Bone quality can be poor. Occlusal forces are high. Crown size and morphology are large, creating emergence profile issues. Placement of a large diameter implant in the molar site offers many logical benefits that can combat these inherent limitations. It can remove the costly need for grafting. It reduces the start to finish time and number of procedures needed. Occlusal forces are better addressed and hygienic troubles can be mitigated. Several reports have shown that the successful predictability of these type implants is high. Although preliminary, our reports appear to share this optimism.
