Trigeminal neuralgia: current concepts of etiology and management

Etiology of trigeminal neuralgia remains a vexed question. There is considerable speculation as to whether the disorder is due to a peripheral or central cause. A functional or organic vascular disorders may play a part. An abnormal mandibulo-maxillary relationship, decayed teeth, and TMJ disturbances may have a deleterious affect upon the trigeminal nerve. Remedial possibilities of trichloroethylene, stilbamidine, dilantin, mephenesin, vitamin B12 and tegretol were discussed. The actual method of treatment must necessarily depend upon interruption of afferent impulses to the high cortical centers. Direct surgical interruption is superior to alcohol injection of the involved nerve.
