With the evolution of orthopaedic instrumentation for internal fixation of the different spinal pathologies, successful outcomes were experienced for conditions where the osseous structures, in which the implants were placed, were normal. Unfortunately, the surgical results for the management of loss of mechanical stability or deformity of the spine with pathological bone were associated with high failure rates, due to frequent loosening or failure of the internal fixation or vertebral reconstruction with cages or cement, resulting in loss of the reduction or reconstruction of the deformity or condition treated. Many of these surgical procedures are complicated, long in duration, with significant blood loss and prolonged post-surgical pain and recovery, which increases mortality and morbidity of these already delicate patients, specially if failure of the osseous fixation occurs.(7,10) Our Spinal Surgery literature confirms that vertebral fractures from osteoporotic bone, due to primary, secondary and carcinomatous bone loss, treated with internal fixation frequently revealed poor outcomes.
