In all cases, we need a meticulously dated history; physical findings which give facts in concrete, objective, and specific form; laboratory data; your observations on functional limitations. This we need in all cases. When the impairment is respiratory, we need, in addition (a) a word-picture of the patient's physical appearance; (b) results of ventilatory tests, if they are available; (c) x-ray findings; and (d) a good picture of the kind of activity that results in shortness of breath.
