Basal leads in temporal epilepsy; electro-anatomoclinical aspects

After a brief premise on the concept of psychomotor and temporal epilepsy, the AA. discuss the most recent contributions, particularly Anglo-saxon and French, that tend explain the function of the foci situated in rhinencephalie structures for the genesis of temporal lobe seizures. From this follows the utility, now admitted by all, of basal lead techniques (in particular those employing sphenoidal electrodes) for an accurate EEgraphic study of subjects affected by epileptic seizures of the temporal type, in order to locate the exact site of the epileptogenic foci. Having accurately examined 21 cases (some of which are described in detail) the authors again stress the advantages, from a practical point of view, of the technique employed by them (two mastoid and two sphenoidal electrodes, one pharingeal electrode) as compared with the common routine derivations; they further discuss some points of peculiar theoretical interest. In this way the problems of the aura, of focal temporal theta waves and of the association of a cortical focus with spike-wave bursts, for each of which a detailed documentation is here reported, are approached.
