A case of acute renal failure resulting from the ingestion of a common proprietary compound containing acetophenetidin, caffeine, and acetylsalicylic acid is presented. The patient developed methemoglobinemia, hemolytic anemia and severe metabolic acidosis mimicking diabetic acidosis. Two hemodialysis were performed before recovery.
