Pathogenesis of fructose hepatotoxicity

Fasted male Wistar rats weighing 350 to 450 g were given 0.55 or 2.2 molar fructose by infusion into a mesenteric vein; control rats received glucose. The effect of intraperitoneal inosine 5'-monophosphoric acid 1.5 mmole/kg 5 min before 0.55 molar fructose infusion was also studied. Fructose loading altered the hepatocellular fine structure. The water content of hepatocytes increased significantly; the increase was dose-related and was accompanied by accumulation of fructose-1-phosphate in liver followed by a sharp fall in ATP. With inosine monophosphate there was a slight drop in ATP. In rats pretreated with inosine monophosphate, fructose loading produced a highly significant increase in fructose-1-phosphate accumulation, a further drop in ATP and an increase in the ultrastructural lesion. Morphological changes induced by the infusions are described and illustrated.
