During recovery from a neuromuscular the sensitivity between various muscles are in the order: pharynx>head and neck>peripheral skeletal muscles>diaphragm. This cause ventilation to return early with restoration of tidal volumes, minute ventilation and maintenance of normocarbia. Later peripheral skeletal muscles return followed by the muscles of the head and neck region. At last, muscles of the pharynx and those involved in the protection of the upper airway recovers. At an adductor pollicis TOF ratio of<0.90 there is a significant risk of aspiration and at a TOF ratio of 0.70 hypoxic ventilatory control is in addition significantly impaired. At adductor pollicis TOF ratios>0.90 patients are unlike to have pharyngeal dysfunction, ventilatory control is normalized and there is an adequate protection of the upper airway.
