Pituitary adenomas (PAs) are benign, slow-growing tumors, and its rare malignant form is pituitary carcinoma (PC), that corresponds to 0.1-0.2% of pituitary tumors. PC is usually diagnosed by metastasis inside or outside the nervous system. This report illustrates a strange case of primary pituitary squamous cell carcinoma without metastasis presenting with the common PA manifestation of mass effect, and thus the challenge of differentiating carcinoma from benign invasive pituitary adenoma, being the former associated to a more severe morbidity and mortality.
