Following the description of three dramatic cases of psychiatric patients with special needs, the article expands on the difficult access to psychiatric care in Puerto Rico, and the beginning of a novel outreach program since 1987. A historical perspective of the beginning of the outreach movement is followed by the description of the keys to success in a psychiatric outreach program. The article ends with great expectations for the future of this burgeoning movement in psychiatry.
