The optimal treatment for patients with coronary artery disease consists of an appropriate blend of life-style modification and medical and surgical therapy. Any patient who continues to smoke, remains obese, and ignores appropriate dietary management or who does not receive appropriate counsel in these areas, is inadequately treated. Patients with coronary atherosclerosis require lifelong treatment, with staged interventions based on symptoms, noninvasive and invasive studies, and assessment of quality of life including employability. Patients with unstable angina, left main lesions, triple and double vessel disease, especially with poor ventricular function and proximal obstructing lesions of the left anterior descending artery, can all benefit from elective revascularization prior to myocardial infarction.
