Mild RHV can explain the prominent anterior forces. Thus, mild RHV due to rheumatic mitral stenosis in combination with the Short P-R, normal QRS syndrome, may best explain this interesting dilemma and vignette. The persistence of her previous symptoms, the short P-R interval and a less prominent R/S ratio in V, postoperatively, favor this conclusion.
