Cardiac catheterization, aortogram and complete surgical repair revealed TF with a patent but narrowed shunt with little flow. Postoperatively, bleeding, pericardial tamponade and complete heart block ensued, with cardiac, renal and respiratory failure. Autopsy confirmed the aboye diagnoses. There were: right atrial (RAH), right ventricular (RVH) and left ventricular (LVH) enlargement, myocardial necrosis, supernumery spleens, absent innominate artery, occlusion of small pulmonary arteries, a closed ductus arteriosus, a bileaflet, thickened stenotic pulmonic valve and a RV patch placed over the moderately severe infundibular stenosis which had been resected.
