We report a 5-year-old girl with a normal chest x-ray early in life who presented with stomach incarceration within the left hemithorax through a congenital posterolateral diaphragmatic hernia. In patients with late appearance of congenital diaphragmatic hernia there is no respiratory compromise and the clinical picture is related to mechanical obstruction of the portion of the gastrointestinal tract herniated through the diaphragmatic defect.
