Diagnostic errors in primary care and in the emergency departments Between 40 and 50 percent (2015) of issues dealt with by the supervisory authorities (lex Maria-issues) from primary care and from acute/emergency departments deal with diagnostic errors. Here, we present underlying issues such as systemic issues, patient related issues and doctor related issues that are more specific in primary care and acute/emergency departments. Finally, we briefly present possible actions to reduce the risk of diagnostic errors.
