People suffering from myocardial infarction require ongoing support during the rehabilitation process. The health education and advice given in hospital may not be appropriate to their needs in the community. Helen Salisbury outlines the potential for health visitor involvement with cardiac patients and their carers following hospital discharge, and describes a suitable model for a health education-led programme.
