A possible form of acute cardiomyopathy as encountered in sugar cane field workers in Tongaat, Natal

An acute form of cardiac failure, responding rapidly to to a potassium-sparing diuretic, is reported in Bantu field workers on the North Coast of Natal. Evidence is offered that it may be a sign of an early stage of cardiomyopathy, and that it is caused by an excess of circulating aldosterone, consequent on a physiological acclimatisation to dehydration, as well as underlying siderotic liver disease.
