Perioperative Blood Glucose Control in Patients with Diabetic Mellitus

Current criteria used for the diagnosis of diabetic mellitus are (1) HbA(1c) level > 6.5% (National Glycohemoglobin Standardization Program certified methods), (2) fasting glucose concentration > 126 mg x dl(-1) (7.0 mmol x l(-1)), (3) 2-h plasma glucose concentration > 200 mg x dl(-1) (11.1 mmol x l(-1)) during an oral glucose tolerance test, or (4) classic symptoms of hyperglycemia or hyperglycemic crisis with a random plasma glucose concentration > 200 mg x dl(-1) (11.1 mmol x l(-1)). It is reported that 15-20% of perioperative patients were with the presence of diabetic mellitus. However, there is little information to guide perioperative glycemic control in these patients so far. In this review, we summarize the current available evidence to assist physician to provide the better glycemic control in perioperative period.
