Reconstruction of massive patellar tendon defects is a challenging problem for orthopedic surgeons. Many surgical techniques have been described, however, there is little consensus on the optimal reconstructive approaches. Most reports in the literature describe techniques dealing with chronic patellar tendon ruptures with associated soft-tissue deficit. This article presents malignant eccrine spiradenocarcinoma of the patellar tendon that required wide excision of the tumor mass, resulting in a 3.5-cm soft-tissue deficit. Subsequent reconstruction of the extensor mechanism of the knee was performed using a combination of an Achilles tendon allograft and semitendinosis/gracilis autograft.
