There is a pressing need for more effective treatment of dyspnea. Although many drugs have been examined, only opiates are of any documented efficacy, and their use is complicated by concern about adverse effects, particularly in patients in whom death is not imminent. A variety of nonpharmacologic strategies have been studied, but most of these have been the subject of only a few, often methodologically flawed studies that enrolled small numbers of patients. When beneficial effects have been demonstrated, the magnitude of the improvement has generally been small and difficult to distinguish from a placebo effect.
