The development of a paper based medical record system and the corresponding database at the Ola During Children's Hospital, Freetown, Sierra Leone are described. This project took place within an extremely resource constrained setting, through the partnership of local and international collaborators. The key factors which define the project are incremental progress, the intent to migrate the data to OpenMRS to ensure its sustainability and building local capacity for database management.
