Intestinal parasitosis during AIDS diarrhoea

This longitudinal study was carried out from October 2000 to November 2001 at the Hepato-Gastro-Enterology department of Gabriel Toure Hospital and at the Department of Epidemiology and Parasitic Infections of Faculty of Medicine, Pharmacy and Odontostomatology of Bamako (Mali) to determine the incidence of intestinal parasitosis among HIV-positive patients with diarrhoea. Faecal samples were tested for the presence of parasites by direct microscopic examination and Henricksen Poblenz Baerman test, Kato Katz test, and PCR. Among the 70 patients involved, the sex-ratio was 53%, and the average age was 35+or-8.4 years. Fever, weight loss, skin manifestations, and digestive mycosis were often observed. Opportunistic infections were relatively frequent, and involved Cryptosporidium parvum (20%), Isospora belli (8.5%), and Microspora (11.5%). Other non-opportunistic microbes were found. Death within the first two weeks were more frequent among patients infected with C. parvum. It is concluded that opportunistic infections among HIV-infected patients require efficient diagnosis and management.
