Access control management in practical settings

Looking at the subject of access control from a practical side, focussing on the electronic health record, we discuss two special cases: the need of providing emergency access, and the possibilities of having a completely automated access control facility. We conclude that the first is essential, and suggest that the latter might not even be desirable.
