A 61-year man showed symptoms of conus medullaris and cauda equina syndrome. MR examination of spine and spinal cord SL section showed changes in medullary cone of ischaemial character. Spinal cord injury was probably caused by thrombosis of spinal cord venous vessels, which developed, with diabetes of the second type, in the result of appendectomy complicated by peritonitis and thrombosis of deep veins of the right shank.
