Ergotism in a patient treated with ritonavir and ergotamine

A case is reported in a 40-year-old HIV-positive man from Spain, who presented with intense pain and coldness in his hands and feet. The patient had been treated with zidovudine, lamivudine, indinavir and ritonavir, and five days before symptoms appeared he had taken an antimigraine treatment of paracetamol, caffeine and ergotamine. Examination revealed peripheral hypoperfusion, and the patient recovered after treatment with prostaglandin analogues. It is concluded that the episode was due to pharmacological interaction between ritonavir and ergotamine.
