We report a case of a patient with right pleural effusion who, during video-assisted thoracoscopy for biopsy and diagnosis, developed a sudden rise in end-tidal carbon dioxide (EtCO2) after a small tear of the lung tissue. The purpose of this case report is to highlight this rare complication and to discuss possible alternative differential diagnosis.
