A 31-year-old woman presented with galactorrhoea nine months postpartum [UK; date not given]. Pituitary magnetic resonance imaging (MRI) showed an enlarged pituitary approaching the optic chiasm with marked diffuse enhancement with gadolinium. A provisional diagnosis of lymphocytic hypophysitis with a differential diagnosis of a pituitary tumour was made. Despite the apparent resolution, however, the patient continues to have significant pituitary dysfunction.
