The index described in this paper is based on the principle that those trabeculae subjected to the greatest tensile and compressive stresses will be the lac.' to disappear as trabecular bone is lost. We have found that this simple and widely applicable method is a valuable adjunct in assessing the severity of bone loss in osteoporosis. A point on the index could be selected that separated, among patients of similar age, those with vertebral compression fractures from those without such fractures with an overlap of only 10.7%.
