The purpose of this study was to clarify the role of nurses in the Department of In-Home Medical Care, a university hospital cooperating with primary care physicians, visiting nurse stations, and health stations in the community. Nurses coordinated between departments in the University Hospital and between the community for patients who were in the terminal stage and/or were provided complex home medical treatments. Nurses' activities in coordination were longer and more frequent than patient education for discharge.
