A special medical editorial looks at translational research, the process of transferring basic scientific findings into clinical practice. For translational research to be effective in the cardiovascular area, a real difference between the healthy and diseased state must be exploited; unfortunately, such instances are rare. The topics examined are gene replacement therapy; endothelial dysfunction, angiotensin-converting enzyme inhibitors, and statins; and the "dark side" of translational research.
