Intrahospital necrotizing pneumonia

A case is reported in a 66-year-old man from Spain , who developed fever and nonproductive cough 5 days after abdominal surgery. Chest radiography demonstrated a cavitary lesion in the middle right lobe and antibiotics were administered for presumed nosocomial pneumonia. Thoracic computed tomography 10 days later showed an increased number of in lesions in the lower and mid-right lobes, and bilateral pleural effusion. Aspergillus fumigatus was isolated from samples of bronchial aspirate. Symptoms resolved after therapy with intravenous liposomal amphotericin B (3 mg/kg daily increased to 5 mg/kg daily) and itraconazole (200 mg/12 h).
