Microbiology and antimicrobials sensitivity of supprative otitis media

Objective: To identify the causative microorganisms of middle ear discharge and their sensitivity to commonly prescribed antibiotics in Primary Health Care clinics at King AbdulAziz University Hospital in Riyadh. Methods: A data collection form included the standard medical history of ear discharge, ear, nose and throat examination, ear swabs culture and sensitivity. A total of 321 patients who presented with the main complaint of ear discharge, from September 1993-March 1995, were included in the study. Results: The most frequently isolated microorganisms were Staph. aureus (26.6%), Pseudomonas species (24.6%), Aspergillus niger (9.2%) and H. influenzae (6.3%). The incidence of Pseudomonas, Proteus and Bacteroids is higher in chronic (more than 3 months duration) than acute infections (i.e. for Pseudomonas 26.7% in chronic and 16.7% in acute cases, Proteus 5.7% and 0.9% and Bacteroids 2.9% and 0.9%). The proportion of sensitivity of the most common microorganisms to different antibiotics were presented. Conclusion: Based on the sensitivity pattern, the inclusion of one of the following Aminoglycosides e.g. Neomycin, Gentamicin and Framycetin as topical treatment is likely to cover the commonest causative microorganisms namely Staph. aureus, Pseudomonas species and H. influenzae. As Aspergillus niger constituted a significant proportion among adult patients, a topical antifungal agent may be of some benefit to this age group.
