Purpose of investigation: Diagnostic and therapeutic approaches in two cases of bilateral massive ovarian edema were evaluated. Methods: Two cases were retrospectively evaluated with the help of the medical reports. Results: Both patients presented with left lower abdominal quadrant pain. Cystic pelvic masses were detected during abdominopelvic examination and confirmed with transabdominal ultrasonography. There were no abnormal laboratory results. Explorative laparotomy was performed with the diagnosis of torsion or rupture of ovarian tumor. Bilateral wedge resection with subsequent frozen section was performed. The final pathology was reported as bilateral massive ovarian edema. Conclusion: A diagnostic bilateral wedge resection with subsequent frozen section is essential in the management and confirmation of the diagnosis. As the vast majority of the cases are young, a conservative surgical management is mandatory to preserve ovarian function.
