Aspergillus osteomyelitis. Report of a case investigated by magnetic resonance imaging

A case is reported in a 55-year-old immunosuppressed man from France who was admitted to intensive care in January 1997 with bilateral lung disease and hypoxaemia. Pseudomonas and A. fumigatus were recovered from bronchoalveolar lavage specimens. Itraconazole (400 mg/day) and amphotericin B (2 g/day) were administered. In May 1997 the patient reported mild pain in his right ankle and investigation by magnetic resonance imaging revealed osteomyelitis of the tibia. A. fumigatus was cultured from a bone biopsy. The patient received itraconazole (400 mg/day) and the bone lesion was curettaged. Three months later the patient was free of pain and his radiological lesions had started to fade.
