Persistent eosinophilic peritonitis associated with fungal infection cured by resection of external Tenckhoff catheter: a case report

A case is reported in a 65-year-old man from Taiwan with chronic renal disease, who developed fungal peritonitis after placement of a Tenckhoff catheter in October 1994. Symptoms resolved after treatment with intravenous fluconazole (100 mg twice daily) and intracatheter amphotericin B, but peritonitis recurred in February 1996 and was treated with antibiotics. In April 1996 symptoms recurred and eosinophilic peritonitis was diagnosed. In July 1996, a white fibrin-like substance was noted in the external part of the Tenckhoff catheter and the dialysate flow became sluggish. The catheter was replaced and microscopic examination of the fibrin clot revealed fungal spores. Symptoms resolved completely after catheter replacement and therapy with intravenous fluconazole (50 mg twice daily) and intracatheter amphotericin B.
