Low CD4 lymphocyte count in a patient with P. carinii pneumonia after autologous bone marrow transplantation

A case is reported in a 5-yr-old girl from Italy who developed fever, dry cough and progressive dyspnoea following a bone marrow transplant. Lymphocyte subpopulation analysis showed that 5% CD4+ lymphocytes with an absolute count of 5/ml. Chest radiography demonstrated bilateral infiltrates and Pneumocystis carinii was identified in bronchoalveolar lavage samples. The patient was treated with cotrimoxazole and steroids, but after initial improvement her condition deteriorated. She died with pulmonary fibrosis but without P. carinii in her lungs.
