The Authors report their experience with the surgical treatment of inoperable tumors of the main bile duct (high portion). They believe that notwithstanding a surgical radical treatment is to aim in every case, and considering that it is extremely rare that demolitive surgery can be completely curative, from the oncological point of view, it may be better to define a specific surgical indication in any single case by guided clinical exams such as liver ultrasound, CT scanning, endoscopic retrograde cholangiopancreatography (ERCP) (and PTC), cholescintigraphy. lt is concluded to extend indications for cholangio-jejunal derivations since with a minor traumatism for the patient it can reach a relatively good survival, not inferior anyway to that following demolitive operations.
