OKT3 is the first of a new class of monoclonal antibodies with therapeutic uses. It is currently the treatment of choice for steroid-resistant acute cellular rejection. The major limitation of the agent is that it can be used only 1 time in a given patient. Although this limitation is generic to the use of monoclonal antibodies, further studies investigating sequential treatment with different monoclonal antibodies having different antibody combining sites may enable repeated treatment with these agents reducing graft loss to acute rejection even further.
