ED is caused mainly by motility disorders in the upper gastrointestinal tract. Prokinetics are therefore a proven therapeutic tool. On the other hand, the gastric mucosa is densely innervated by sensory vagal and sympathetic fibers. These are also responsible for the mediation of discomfort and pain. The "underground" diffusion of acid through an internal leak developed in a damaged mucosa might be capable of triggering ED. This could explain the occasional effectiveness of acid blocking in this type of discomfort.
