52 women treated at a hospital in Madrid between June 1980 and December 1984 were included in this study of the epidemiology of cervical intraepithelial neoplasia (CIN III) in Spain. Diagnostic procedures in all cases included vaginal cytology, colposcopy, and biopsy under colposcopic control. A control group was constituted of 102 women seen in the same practice within the same dates who were free of CIN III. The averages age of CIN III patients was 37.2 years, with a peak incidence between 35.45 years. 27% were nulliparas and 2.5% has 4 or more children. CIN patients has a slightly increased incidence of infection by the human papilloma virus, but the difference was not statistically significant. 6.4% of CIN patients initiated sexual relations before 15 years of age and 38.3% between 16-20 years. The relative risk of CIN is 2.5 when sexual relations began before 15 years and 3.35 when they began before 20 years. Development of CIN was not associated with use or oral contraceptives but was significantly associated with use of IUDs. The relative risk of CIN was 2.77 for IUD users. Marital status was not related to CIN. More frequent diagnosis of CIN in patients of high socioeconomic status was believed to be related to their tendency to seek health care more frequently than younger women.
