We report a case of a cystic esophageal duplication with primary cardiac symptoms and bronchogenic cyst in an adult. Duplications of the esophagus are often operated on the diagnosis of mediastinal tumor. With new imaging techniques the diagnosis can be guessed before operation. Nuclear magnetic resonance and endoluminal ultrasound are two newer techniques which are valuable in assessing "mediastinal" tumors.
