Peritonitis due to Wangiella dermatitidis in a patient on CAPD

A case is reported in a 39-yr-old man with renal disease secondary to chronic glomerulonephritis who had received continuous ambulatory peritoneal dialysis (CAPD) for 3 yr. The patient presented with abdominal pain and cloudy dialysis fluid. W. dermatitidis was cultured from peritoneal fluid and from swabs of the inner walls of the chamber of the patient's UV exchange device kit where spiking of the dialysate bag occurs. The Tenckhoff catheter was removed and the patient was treated with oral fluconazole for 2 wk. Questioning of the patient revealed that he had not serviced his UV exchange device for >2 yr.
