Corpus luteum function assessed by serial serum progesterone measurements after laparoscopic endotherm sterilization

Estimation of serum progesterone as an indication of luteal insufficiency after endotherm sterilization is reported for 14 patients aged 29-43 years (mean age, 35.2 years). All patients were regularly menstruating, and none was using hormonal contraception. Serum samples were collected from study subjects on days 17, 19, 21, 23, and 25 of the menstrual cycle preceding sterilization. Control serum was collected on corresponding days during the 2nd normal cycle after sterilization. Serum progesterone values on the 17th day of the cycle were significantly larger after endotherm sterilization. Otherwise, no statistical differences in values between the 2 series were noted. However, a nearly significant reduction in values was observed on the 23rd day in the post-sterilization series. This could indicate a shortening of the luteal phase by an earlier maximum luteal function followed by a relative luteal insufficiency. In response to a questionnaire on experiences in the postoperative period, 6 of 12 patients reported changes in menstrual regularity and 4 had increasing dysmenorrhea. The tendency reported to a shortening of the length of the menstrual cycle could be due to a luteal insufficiency, to a shortening of the follicular phase, or to both. The fact that even this atraumatic method of sterilization can disturb ovarian functioning should be pointed out to sterilization seekers.
