The saturation dose for a child with heart failure or tachycardiac rhythm disturbances is 0.03 mg/kg for Dilanacin "neu" and 0.04 mg/kg for digitoxin. The maintenance dpses are 0.01 mg/kg/day for Dilanacin "neu" and 0.003 mg/kg/day for digitoxin. This is 1 droplet of Digitoxin liquidum per kg body mass and day (1 ml=30 drops=0.1 mg). Deviations of sodium, potassium or calcium levels must be corrected. Diuretics used by the author are furosemide (1-3 mg/kg/day) and additionally Verospiron (2-3 mg/kg/day). Successful antifailure therapy often is the prerequisite of cardiac catheterization and cardiac surgery. Adequate doses of glycosides may be continued over months or even years in patients with inoperable congenital heart disease. After successful repair of congenital heart disease the respective drugs may soon be discontinued.
