Scabies [caused by Sarcoptes scabiei] remains a significant source of morbidity in residential nursing homes for the elderly because of its highly contagious nature. This article describes the clinical presentation, including atypical manifestations, and treatment. Nursing home protocol is then discussed using a step-wise plan in which patient diagnosis, patient treatment, decontamination of the patient's room, evaluation of patient contacts and follow-up procedures are all assessed. Although a number of scabicides are available, the authors recommend the use of permethrin cream for treatment of elderly patients.
