This paper addresses conceptual issues underlying the assessment and implementation of health care priorities in developing countries as practised by foreign development agencies coping with a potentially destabilizing unmet social demand. Health care priorities that aim to eradicate the diseases of under-development must address the structural causes of under-development rather than defending capitalist modes of production and the resulting poor social conditions of the mass of the people.
