Differential diagnosis of convulsions and epilepsy in children

Starting from the factual difficulties with which the pediatrician is faced, the authors analyze an important aspect in the approach to children with paroxysmal cerebral manifestations, namely the epileptic or non -epileptic nature of these manifestations. The value of this differentiation resides in the therapeutical, psychologie and social consequences for the patient when epilepsy is diagnosed. Other causes of loss of consciousness are reviewed (convulsivant and nonconvulsivant syncopes, spasm of a burst of crying, diverse vertige variants, hypersomnia, nocturnal paroxysmal manifestations - pavor nocturnus, sleep walking, nocturnal verbal automatism, masticatory automatism, sleep paralysis, the "Alice in Wonderland" phenomenon, nocturnal enuresis etc.), paroxysmal psychical manifestations, rhythmic motor manifestations of the head (nutans spasm, jactatie eanitis nocturna, ticks etc.), benign infantile myoclonia, choreic movements, attacks of fear, masturbation, visceral paroxysmal manifestations, migraine, tonic nonconvulsivant fits, paroxysms specific of the neonate "fictitious" convu sions and other nonconvulsivant attacks. The authors show the clinical, electroencephalcgraphic elements and other Jiata furnished by paraclinic exploration which permit a correct diagnosis and - the institution of an adequate efficient. therapy.
